Did you know that we have a Citizens Advice Bureau advisor in the practice on Fridays who can offer free, confidential, impartial and independent advice to patients registered at the practice. Patients can self refer- Please speak to Reception.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your previous medical records. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.


Please print off:

1)   Registration Form

(Please note: If you have previously registered with a NHS GP. We will need your NHS (National Health Service) number, if you do not have this please contact your previous GP)

2)   Enhanced Data Sharing Model Consent leaflet


3)    Adults Questionnaire   or      4)   Childrens questionnaire

Please bring all completed forms to reception to begin the registration process, along with:

  • photo ID (passport, driving license etc)

  • proof of address (utility bills etc)

  • and any record of immunisations.

  • If you are visiting from another EEA

    Please bring with your current EHIC (or Provisional Replacement Certificate (PRC))/S1    Information for visitors from abroad.

Where possible, please return them to reception before 5pm.
